Reporting an Incident of Discrimination or Harassment
The Duke Office of Student Conduct makes available an online form for reporting incidents involving students and/or student organizations. Anyone with a Duke NetID may use this form to report an incident.
Upon receiving a report, the Office of Student Conduct staff will respond as appropriate. You will be asked to authenticate your identity before being directed to the online incident report form. If you have any problems accessing the form or authenticating your identity, please contact the Office of Student Conduct.

Counseling and Psychological Services (CAPS)
CAPS provide a wide range of services, all covered through the Student Health Fee, that meet the mental health and student development needs of Duke students. At CAPS, they address a wide range of life issues concerning a student’s mental health, not just mental health emergencies.
