You will login to the CKD dashboard (directions here CKD Dashboard Project.pptx). Make sure to select your panel of patients and not all of the fellows clinic. First year fellows make sure to include your panel and the graduating fellow you took over for.

You will utilize the following fields in the CKD dashboard for your evaluation of your panel:
