Email contact Info: VHADUR CAC (VHADURCAC@med.va.gov)

How to Request a Note to be Hidden:

*Extra step is involved if the note is associated with an image, see “Removing an Image” below

*In most cases the author of the note must make the request (unless you are the supervisor)

  1. Make an addendum to the note requesting that the note be hidden, and why.
    1. If you want only an addendum to a note hidden, be sure to identify the addendum by the time it was signed
    2. If the main note has existing addenda, be aware all addenda will also be hidden.
    3. If you are not the author of the main note, the author needs to make an addendum concurring with the request to hide the note, and to ask for their addendum to be hidden.
  2. Sign the addendum requesting the note/addendum to be hidden.
  3. Hover/right click over the signed addendum and select Identify Additional Signers (not Cosigner)
  4. Select a couple of Clinical Application Coordinators to notify (in case some of us are away): Joshua Stiles, Lisa Klatt, LeAndria Dingle, Som Chaleunvong, Richard L Anderson, Janet R. Brown, Sonny Roaquin
  5. We will receive a notification when we log into CPRS, please do not email or call us.
  • Create an addendum
  • Select Identify Additional Signers
