Connor Drake, MPA

Connor is a Research Program Leader at the Center for Personalized Health Care at the Duke University School of Medicine and a Health Policy and Management PhD candidate at the UNC Gillings School of Global Public Health. Currently he is involved with efforts to promote personalized, proactive, and patient centered models of care in the Veterans Health Administration and the Lincoln Community Health Center and developing strategies for health systems and community partners to respond to social determinants of health. His research interests are focused on population health management, implementation of evidence-based interventions to improve health self-management, and clinical care redesign to engage patients to address medical and non-medical needs.
