The Advance Care Planning

and Healthy Living Through Faith Program

We are a community of academics, clergy, and clinicians working together to explore how we can live well and die faithfully as both, spiritual and physical beings

We are a committed community of experts working together to bridge the gap between the spiritual and medical understandings of end-of-life care.  We strive to provide learning resources for African American clergy and churches about the basics of advance care planning, palliative care and treatment, and hospice services, along with creating a centralized database of end-of-life resources for academics, clinicians, and families.

Through the African American Advance Care Planning/Palliative Care Network (the “AA ACP/PC Network”), a group of advance care planning and palliative care experts advocating for greater access to and participation of African Americans and other communities of color in end of life care, we plan to address the most pressing research questions being pursued or needing pursuit on the health disparities affecting modern health care, and will identify key opportunities for cross-disciplinary research collaboration.
The John and Wauna Harman Foundation is a small, private family foundation focused on improving end-of-life care in the U.S., and encouraging end-of-life planning conversations for all Americans.
