“Population Health: An Informal Conversation with National Leaders” Event Coming April 20

Duke Community and Family Medicine will host “Population Health: An Informal Conversation with National Leaders” from 5 to 6 p.m. April 20 in the Learning Hall of the Mary Duke Biddle Trent Semans Center for Health Education. In this event, national leaders in population health from A Practical Playbook will speak about how collaborative partnerships between primary care and public health can lead to improved health outcomes. They will also speak about The BUILD Health Challenge and other initiatives that support collaboration between public health and primary care.

Leading the conversation are steering committee members from A Practical Playbook:

  • Lloyd Michener, M.D., professor and chairman of Duke Community and Family Medicine (moderator)
  • Ed Hunter, MA, chief executive officer of the de Beaumont Foundation (introduction)
  • Denise Koo, M.D., MPH, senior adviser for health systems at the Office of Public Health Scientific Services, Centers for Disease Control and Prevention
  • Brian Castrucci, MA, chief program and strategy officer at de Beaumont Foundation

A Practical Playbook is a joint project developed by the de Beaumont Foundation, Duke Community and Family Medicine, and the Centers for Disease Control and Prevention (CDC). A Practical Playbook is a stepping stone in the next transformation of health, in which primary care and public health groups collaborate to achieve population health improvement and reduced health care costs.
