What is PRISM?

A broken bone is a signal that you are at risk for more serious injuries.

After breaking a bone, adults are about twice as likely to break another bone or have a fall-related injury within the next year.

A healthy diet & exercise are important, but are usually not enough to prevent more breaks.

Certain medications have been shown to strengthen bones and make them less likely to break again.

Reducing medications that cause dizziness, sleepiness, or make you unsteady can help avoid falls & injury.

Even if you don’t notice these symptoms, reducing medications can help.

A PRISM nurse consultant, supported by an interprofessional team and working remotely, will provide recommendations, shared decision making, and support to patients both in the skilled nursing facility and after they transition home.

Patients will receive one of the following evidence-based care models:

The main questions this study aims to answer are:

Do patients in skilled nursing facilities who receive a PRISM care model have fewer falls with injuries than those who don’t?

Are important symptoms like pain, depression, anxiety, sleep, fear of falling, and medication side effects different in patients receiving the different care models?
