About PRISM

PRISM is a program to optimize medications to prevent falls and injuries in patients receiving rehabilitation in a nursing home after a hospitalization for a broken bone.

PRISM is a collaboration between the Duke University School of Medicine, Harvard Medical School/Hebrew SeniorLife, and nursing homes across the country. This project is supported through a Patient-Centered Outcomes Research Institute (PCORI) Project Program Award (OFP-2022C3-30363).
