Interviewer: Could you talk a little bit more about how the care that you provide has been affected by the Dobbs decision?
Anonymous M.D.: I think the biggest difference and challenge that I have noticed, are people having to travel even further in order to get the services that they could have had in their home state previously. It is a lot to travel six hours to come to get the services that you need. I have seen patients recently that have traveled from Florida, or that have traveled from Georgia, or even further away. I say, “How long did it take you to get here?” They have to incorporate travel cost, time, finding a place to stay overnight usually, especially if they are further along. They may have to have a laminaria or cervical prep the day before, and so they would need to stay overnight. Those are things that were maybe…things that came up rarely before, but now I feel like every time I am in clinic, I am helping facilitate care for someone who is out of the state of North Carolina. There are more surgical procedures that are happening in the OR because patients are further along because either there is a ban on all abortions in the state that they live in, or it’s a lower gestational age than it was previously, and so we are seeing a lot of patients who are a little bit further along potentially because that service isn’t offered in the state. But also, it takes them time to gather the resources in order to go somewhere to be taken care of, and so weeks can pass while you are trying to figure out where you are going to stay, what clinic you’re going to be able to go to, and so I do think we are doing more surgical procedures, or procedures in the OR, because people are coming from further away. Also, if you’re having a procedure, it all happens in one to two days, versus you needing to follow-up if you take mediation. Follow-up is typically an ultrasound, so will you be able to get an ultrasound in your home state for follow-up. It would sometimes be hard for you to take medications, and then come back for follow-up in a week or two to see us. There has been more facilitating care outside of you being here in the clinic with us and us taking care of you.
