Identifying the “right” behavioral/mental health treatment options can be complex for patients given insurance and provider limitations.
For patients with PHQ-9 score of 10-19 (mild to moderate) with a diagnosis of depression and/or an anxiety diagnosis—GAD7 ≥ 10, they may qualify for Collaborative Care Management
Who can help:
- Discuss with DOC Team Social Work (Jan Dillard, Elissa Nickolopolous) or Greg Brown, prior to providing patient with behavioral health resources. They will guide you with appropriate information for the patient and for the after visit summary
- If the Social Work team agrees that the patient is a good candidate for Collaborative Care Management, they will let you know and generate a referral for you to sign. For more information on Collaborative Care, please look here: Duke WELL Collaborative Care Model flyer
- If neither Team Social Work nor Dr Brown are available in clinic, don’t hesitate to order an ‘ECommunication DOC Behavioral Health‘ for further review and recommendations.
