Proposal Abstract:

The goal of the Achieving Healthy Bodies and Healthy Minds for a Lifetime (AHL) Team is to reduce the incidence and prevalence of obesity for all residents of Durham County. The AHL team will achieve our goal by improving diet and increasing activity; our model will address the problem from family, neighborhood and cultural perspectives crossing age, racial, socioeconomic and gender divisions, thereby reducing both health disparities and healthcare costs in Durham.

We will work with The Durham Public School System, community health care providers (both those in private practices and in university and community clinics), faith-based institutions, childcare centers, neighborhood associations, and community agencies to create partnerships between the community and Duke that will allow each community to contribute to the decisions made in their community creating ‘ownership’ of the program and coordinate and tailor the health information while expanding health opportunities, thus generating programs specific to each neighborhood.

We plan to test this program in a few pilot communities then proceed via a phased roll-out throughout Durham County.
