Proposal Abstract:

Improved cancer screening holds promise to reduce cancer deaths in Durham County. To improve cancer screening, we will identify at-risk individuals, remove barriers to screening, and improve early-detection through efficient utilization of state-sponsored cancer screening/treatment resources. We propose a three-tiered program for:

1) cancers with validated screening and state-funded programs (breast/cervix cancers)
2) cancers with validated screening but no state-funded screening program (prostate cancer and colon cancer), and
3) cancers with no validated screening (lung/ovary/pancreatic cancers).

We will use geo-spatial mapping and community focus-groups to identify existing gaps and populations in need of improved screening, develop computerized models to assess cancer-risk for an individual, and generate tailored cancer screening plans linked to an integrated health care record. We will develop coordinated informatics/data sharing and Duke-Durham community research-partnerships. Our health-network will initially focus on women’s cancers but can serve as a model for comprehensive cancer prevention and whole person wellness. Members of the Duke Comprehensive Cancer Center Cancer Control and Prevention Program, including Isaac Lipkus, Marva Price, and Victoria Seewaldt will work to integrate the aims of this project with ongoing risk reduction strategies.

To improve access, health education and raise community awareness about the importance of cancer screening, we will expand our highly successful Patient Navigator Program, to focus on whole-person health, rather than just breast cancer. Our Patient Health Navigator will 1) register Durham residents through community clinic sites, health fairs, and pre-natal visits, 2) evaluate individual and family cancer risk, and 3) increase compliance with screening and follow-up. Our expanded Patient Health Navigator Program can serve as a training resource for programs within Duke and throughout Durham County.
