- Uninsured (no Medicaid, Medicare, VA insurance, or private insurance)
- Medicaid Family Planning is eligible
- Medicare Part D Coverage Gap is eligible
- Income at or below 300% of Federal Poverty Line

- Speak to Jan or Elissa to screen for eligibility.
Senior PharmAssist
Eligibility:
- Age 60 or older
- Live in Durham County
Services:
- Medication therapy management (geriatric clinical pharmacists who provide recommendations about medication): up to 300% of FPL
- Direct financial assistance (pharmacy cards with secondary coverage to a Medicare-approved drug plan): up to 250% of FPL
- Medicare insurance counseling (counseling about choosing the right medical or prescription coverage plan): any Medicare beneficiary in Durham County
To refer:
