Social Work Services
- Safety assessments and referrals/reports: SI, HI, domestic violence, abuse/neglect
- Brief behavioral interventions at the time of appointment for patients whose problems are related to behavior (for both physical health and mental health)
- Comprehensive biopsychosocial assessments and chart review: detailed work history, substance use history, mental health history, funding, emotional support, coping strategies and personal strengths/resources, ability to access medications and medical services
To refer:
- Your first choice (and never the wrong answer) is to find Jan Dillard, LCSW or Elissa Nickolopolous, LCSW in the back hallway on the Lavender side.
- Or use Secure Chat in Epic (speech balloon in upper left corner) to ping them.
- Or use InBasket in Epic to send a referral message using the dotphrase .SWDOCSWREFERRAL.
- If a patient has been coming to the DOC for a while, and especially if dealing w/ a chronic issue, type “LCSW” in the Search box in the patient’s chart and it should pull up prior Social Work notes, so you can double-check if the issue has been addressed previously (maybe even last visit).
- Note: EComm DOC Social Work has been discontinued.
Chaplaincy Services
- One-on-one Chaplain Services at the DOC
- Accepts referrals from providers and staff
- Supporting staff
- DOC Grief and Spirituality Group meets weekly in the Fall and Winter, monthly in the other seasons
- Speak to/InBasket/Secure Chat Katherine to refer patients on a rolling basis.
