DOC is home to multiple specialty clinics, including the following (in alphabetical order):

Addiction Medicine (Dr. Meredith Niess) – Friday AM

  • Services: medical treatment of addictions including opioid use disorder, alcohol use disorder, stimulants, etc.
  • To refer: place a check-out note for “DOC addiction medicine”.

Cardiology (Drs. Monique Anderson, Akshay Pendyal) – 2nd/4th Thursday PM (Anderson) & 1st/3rd/(5th) Wednesday AM (Pendyal)

  • Services: general cardiology services, including (but not limited to) CAD, CHF, arrhythmias, HTN/HLD evaluation and management.  Would encourage referring to DOC Cards as “first stop” for Cardiology, esp. for patients facing barriers to attending appointments.
  • To refer: place a check-out note for “DOC cardiology”.

Diabetes Team (Dr. Diana McNeill and team) – 1st and 3rd Tuesday PM

  • Services: management of patients not already seeing endocrinology with uncontrolled DM and added social issues or complexity. The team also includes Valerie Keck NP, Holly Canupp PharmD CPP, Jan Dillard LCSW, and Elissa Nickolopoulos LCSW.
  • To refer: place a check-out note for “Dr. McNeill Tuesday DM clinic”. Do not use the term “endocrine” to avoid confusion.
  • This team clinic takes place on the first and third Tuesday PM of the
  • Refer patients in the “check out” section of the encounter Wrap Up – our front desk can Don’t use the word endocrine, to avoid confusion with the endocrinology clinic! Say “Dr. McNeill Tuesday DM clinic” or similar. (for Maestro bonus points, make it a speed button by clicking the wrench in that section).

Endocrinology (Dr. Tracy Setji) – every Wednesday PM

  • Services: general endocrinology services. The team also includes fellows and residents.
  • To refer: place a check-out note for “DOC endocrinology”.
  • If your patient has urgent Endocrinology issues and there is limited clinic availability, send an in-basket message to Dr. Setji and she can help facilitate care.

Mobile Eye Van – seasonal, every 3rd Tuesday all-day

  • Services: FREE diabetic retinopathy screening exams
  • To refer: place a check-out note for “DOC mobile eye van”.

Interprofessional Education

  • Several providers at the Duke Outpatient Clinic have created a standardized approach to providing interprofessional education for our students. More information can be found in the DOC Interprofessional Education Curriculum Project within the DOC’s folder in the Duke Box.

Physical Therapy (rotating) – every Friday

  • Services: general physical therapy services, including on-site treatment of neck/back pain, shoulder/knee injuries, pre/post-surgical management, sports rehabilitation, arthritis conditions, traumatic injuries, overuse/repetitive injuries.
  • To refer: place a check-out note for “DOC physical therapy.” Please prioritize patients with transportation or insurance barriers and refer others using Amb Referral to PT
  • On-site conditions treated include: neck pain, back pain, knee injuries, shoulder injuries, pre-surgical management, post-surgical management, sports rehabilitation, arthritis conditions, traumatic injuries, overuse/repetitive
  • DOC physical therapy should just be for the above orthopedic conditions.  All other PT needs listed below, refer using “Amb Referral to PT,” and specify the service needed
  • In addition, a variety of physical therapy services are available throughout the health system. These include gait and balance training, rehab for orthopedic conditions, post- surgical rehab, cardiac and pulmonary rehab, vestibular rehab, neurological rehab, pediatric therapy, women’s health therapy (pelvic PT), and aquatic
  • Besides physical therapy, Duke also offers speech therapy and occupational therapy which can include mobility evaluations for wheelchair needs, hand therapy, and ADL

In short, think of DOC PT as prime real estate reserved for those with the biggest barriers to accessing care.

Psychiatry (Dr. Greg Brown)

  • To refer: place an order for “E-communication to DOC Behavioral Health Referral/Review”.
  • Brown is our Med-Psych attending who specializes in medically complex psychiatric patients and is available to see patients for consultation or short-term follow-up with several types of conditions. The Med-Psych attending can also provide treatment recommendations through a collaborative care framework.
  • All referrals and management questions for DOC Psychiatry (Dr. Greg Brown) are placed through an E-Communication consult to DOC Behavioral Health Referral/Review (Duke Outpatient Clinic only). Our hope is to better screen and route patients to the appropriate level of care and improve
  • Patients MUST be referred by their PCPs/DOC Providers (no self-referrals).
  • The E-Communication is a templated “e-consult” that reviews a patient’s case/chart to share information and advise care. This may include scheduling an in-person consultation appointment or referral to other
  • With completion of the E-Comm, recommendations will be outlined in the E-Comm Medication changes and/or new medications as well as diagnostic studies will need to be ordered by the referring provider or PCP. (There may be situations where Dr. Brown can assist with these orders but ongoing communication with the referring provider will be key.)
  • If there are follow-up questions, please place another E-
  • Once patients are psychiatrically stable, their care will be transferred back to the PCP
  • If there are ever any questions about the appropriateness of referrals, please feel free to contact Brown directly, even in real time.

Smoking Cessation (Dr. James Davis, Bolu Abe-Lathan PA-C)

  • Services: comprehensive evaluation of which smoking cessation treatments will be most effective, including evidence-based medications and behavioral treatments; long-term phone-based “check-ups”; access to research studies.
  • To refer: place an order for “Ambulatory Referral to Smoking Cessation Program” and select Duke Outpatient Clinic as the site.

DOC Quit At Duke Smoking Cessation Program (Clinic at DOC)

  • Use “Ambulatory Referral to Smoking Cessation Program” (919-613-QUIT)
  • Comprehensive evaluation to determine which treatments will be most effective
  • Evidence-based medications — often combination medications or adaptive treatment
  • The option of several evidence-based behavioral treatments
  • Long-term phone-based “check-ups”
  • Access to research studies

Weight Management (Dr. Fatima Syed) – Thursday AM

  • Services: medically-supervised weight management services, including medication-assisted, low-carb, etc.
  • To refer: place a check-out note for “DOC obesity clinic”.
  • Note that many of the handouts are available in the Obesity Medicine Handouts section here: Forms and Patient Handouts
  • For motivated patients, their comprehensive clinic teaches the low carb (i.e. ketogenic diet) and has successfully had many patients control their diabetes and even reduce/eliminate their insulin requirement.
