Completing request for Personal Care Services (PCS)?
For patient with regular Medicaid (including dual Medicare-Medicaid) who needs help with activities of daily living (ADLs)? Here is link to electronic version of form; we also have hard copies in folder in preceptor room and in both nursing stations. (NOTE: The date should be 7/1/2021 in lower lefthand corner; outdated forms will not be accepted.)
BUT if patient is covered by one of the named managed care Medicaid plans (Carolina Complete, Healthy Blue, Wellcare Medicaid or Amerihealth Caritas; we don’t talk about UHC), each has their own PCS form. HEALTHY STEPS
Part of Root Causes, the Duke medical student-led organization that provides fresh produce to DOC patients and helps newly housed individuals, Healthy Steps helps patients create their own exercise goals and pairs them w/ Duke health professional students who can serve as exercise partners. How to Refer:

- Your first choice (and never the wrong answer) is to find Jan Dillard, LCSW or Elissa Nickolopolous, LCSW in the back hallway on the Lavender side. Or use Secure Chat in Epic (speech balloon in upper left corner) to ping them.
- If a patient has been coming to the DOC for a while, and especially if dealing w/ a chronic issue, type “LCSW” in the Search box in the patient’s chart and it should pull up prior Social Work notes, so you can double-check if the issue has been addressed previously (maybe even last visit).
Note: EComm DOC Social Work has been discontinued.

Medicare Plan Assistance
Gurleys Pharmacy is offering assistance for patients during Medicare open enrollment. They have hired a person specifically to help patients get the best plan suited for the individual (this person may be available after the enrollment period as well). There will be 1-hour appts at the pharmacy. Call Gurleys and give them the patient’s name/birth date/phone#. They will call and set up an appointment for them. They do not need to be a Gurleys customer. Tara (at Gurleys) said that the plans this year are better. More of them have extra benefits like pre-loaded cards to help pay for OTCs and free gym memberships. Many have lower co-pays. Gurleys Pharmacy phone # 919-688-8978

Durham Ensure Program
A new program from Durham Center for Senior Life provides Ensure to Durham residents 55+ the first Friday of every month! Recipients have to come to the DCSL offices at 406 Rigsbee and can receive up to 2 cases; all will be asked to sign up for a free DCSL membership. Pickup is from 10a-noon, and first-come, first-served based on priority (post-hospital or facility discharge w/ doctor’s note; current DCSL day health participants; disabled seniors, etc.).

Caregiver Support Program
The Duke Caregiver Support Program offers free support around caregiver needs; you do not need to be a Duke Health patient to receive these services.
The program helps connect caregivers to verified service organizations and resources at Duke Health and in our local community, and provide guidance on home care and placing loved ones into long-term care communities, as well as emotional support and education. Click here for more info.
Dementia Care Management
DCSL is starting a new program for individuals living with dementia or cognitive impairment and who live alone in Durham. Hope4Healers Helps Health Professionals Cope
The Hope4Healers Helpline (919-226-2002) is a new initiative in partnership with the North Carolina Psychological Foundation. It provides mental health and resilience support for health care professionals, emergency medical specialists, first responders, other staff who work in health care settings and their families throughout the state who are experiencing stress from being on the front lines of health care.
Hope4Healers is also available 24 hours per day, seven days a week for people to reach out for support from a licensed mental health professional.
Caring For Each Other Fund
At DUHS, we all play a role in lifting each other up and bringing compassionate care to life. A prime example is the Caring for Each Other Fund, created to help team members who are facing unexpected hardship. By providing financial assistance with basic living needs during a crisis or emergency, we aim to lessen their stress, help them weather the storm, and return to financial health and stability.
The fund helps eligible DUHS team members with up to $1,500 for emergency, unexpected financial hardships triggered by qualifying events; apply here. (If you would like to give to the fund, click the Donate tab.)
HELP Is Just Across The Parking Lot
HELP (Health Equipment Loan Program) is now our neighbor! That means we (and our patients) are so much closer to rolling walkers, canes, bedside commodes, and so much more! HELP was founded by retired Duke RN Anne Drennan and Duke complex case manager Gayle Weaver and now calls Project Access of Durham County home.
