NEW DIABETES PATIENT RESOURCE

DukeWELL eligible patients with diabetes will receive a new form of patient education through MyChart. This Care & Lifestyle Resource Center aims to engage patients by providing additional resources and education, including:

      • recipes of the week
      • exercise information of the week
      • a suite of education videos and articles
      • connections to programs in their community
      • connections to offerings at Duke

Approximately 30,000 Duke Patients with diabetes were notified when the resource center was launched. Many of the resources are also available in Spanish. FIT FEET PROGRAM 

Fit Feet is a resource for low cost toe nail trimming for those age 55+. PCP referral form is required, and cost is $30 for those who live outside of Orange County ($15 for those who live in the county). It is usually >$80 at the podiatrist, so this is a good deal! See more details here.


FREE DM RETINOPATHY SCREENING

DON’T FORGET that FREE retinopathy screening is available every month at DOC through an on-site Duke Eye Center van. Just put “diabetic eye screening van at DOC” in the check-out note to schedule. Here is a handout to share with patients about screening.


CGM ORDERING PROCESS DO’S/DON’TS  

We need your help when ordering Freestyle Libre and Dexcom continuous glucose monitors. 

DO: Check the CGM board in the Precepting Room for insurance-specific instructions and handouts.  Ask Holly Canupp or Val Keck for guidance.

DON’T: Don’t send CGM orders for patients with Medicare to their pharmacy. This is incorrect. CGMs are considered DME—which means a separate ordering process.
