Mental Health Access and Coordination Initiation (MH-ACI) Breakthrough Series Collaborative
MH-ACI is composed of five teams, each with six to 12 team members, representing clinics within the Durham VAMC. The Collaborative completed its first Learning Session in February 2011
Goals
- Access to mental health care services as a component of overall health care
- Coordination of mental health services and integrations with primary care
- Development of a veteran-centered mental health care model that effectively engages veterans and their social support network in health care decisions
