VA Mid-Atlantic Health Care Network (VISN 6) Ambulatory Care Services Optimization Project (ACS-OPT) Breakthrough Series Collaborative
VISN 6 is one of 21 Veterans Integrated Service Networks (VISNs) of the Veterans Health Administration, Department of Veterans Affairs (VA). VISN 6 is composed of eight VA Medical Centers (VAMCs) and 18 community-based outpatient clinics across Virginia, West Virginia, and North Carolina. ACS-OPT Collaborative Teams have been continually generating, testing, and evaluating small tests of change (STOCs) since late May 2010. As a result of the project’s many successes and in anticipation that the Learning Collaborative objectives will be accomplished, the VA has extended the ACS-OPT for nearly a year to include an additional Learning Session and Action Period in late 2011.
Goals
- Improve the quality of health care services provided to veterans, beginning with ambulatory care
- Build the organizational capacity needed to implement, sustain, and continue to improve care delivery across VISN 6 clinics and medical centers
- Rapidly deploy organizational and practice changes that are shown to produce breakthrough results in outpatient primary care
