Initiation and Planning Phases

During Initiation and Planning Phases, Duke EPIC staff members work collaboratively with partner organizations and topic experts to: assemble a Leadership Team, draft a Collaborative Change Framework, recruit and train Collaborative Faculty, recruit and enroll Collaborative Teams, and lastly conduct Collaborative Launch activities.

Monitoring and Assessment Phases

Monitoring and assessment are critical to ensuring that evidence-based practices are implemented with fidelity and that practice changes can be maintained over time.  During monitoring and assessment phases Duke EPIC improvement advisors and program managers work with Collaborative faculty members to coordinate Learning Sessions and Action Periods that include activities to monitor and assess Collaborative progress.

Outcomes

Following completion of the all Collaborative activities and receipt of all required metrics, the Duke EPIC team works with Collaborative leadership and faculty to produce a Final Report, detailing the work of the Collaborative and summarizing the outcomes assessed and lessons learned.
