As an initiative of Heart Tones, a literary arts ministry founded in 1986 by Dr. Gloria Anderson, ACP for AA is a nationwide educational outreach program partnering with organizations to help bridge the racial disparity gap in healthcare delivery, and promote healthcare decision-making through advance care planning conversations, education, and preparation that help bridge disparity gaps for Black Americans.

Detroit and Flint, MI

An affiliate of the Michigan Center for Urban African American Aging Research (MCUAAAR), ENGAGE US fosters community trust and engagement around research through free educational and health programming to ensure older African Americans in Detroit and Flint, MI, are better represented as participants in health research. The Recruiting and Retaining Older African Americans into Research project (ROAR) and the Healthier Black Elders program (HBE) are two current initiatives developing sustainable national models to increase the representation of Black older adults in health research in a way that elevates the voices of African American older adults.

Ann Arbor, Detroit, and East Lansing, MI

Founded in 1988, this collaborative between Wayne State, Michigan State, and the University of Michigan focuses on community outreach, academic research, and faculty mentoring.  It is one of eighteen national Resource Centers for Minority Aging Research (RCMAR) and specialized Alzheimer’s focused projects funded by the National Institute on Aging.

Established in 1976 at the University of Michigan’s Institute for Social Research, the PRBA has become a leader in using new and innovative qualitative and quantitative research methods to understand the lives of African American and African descendant communities.  It continues to be a center of academic scholarship, research, and training for social scientists focused on the Black family, social support and religious affiliation and participation.

This nonprofit organization is a national network of professionals, volunteers, and institutions that raise awareness about the needs of children and teens who are grieving a death and they provide education and resources for anyone who supports them. Under the tenet that no child should grieve alone, no matter where they live or their circumstances, this organization provides educational programming and critical resources to facilitate mental, emotional, and physical health so children can positively adapt to the loss in their lives.

As a founding member of the International Alliance of Carer Organizations (IACO), the Alliance conducts research, develops national best-practice programs, and works to increase public awareness of family caregiving issues in more than 80 state and local caregiving coalitions.

This program of the National Hospice and Palliative Care Organization (NHPCO) provides free resources to educate and empower patients and caregivers to make decisions about serious or life-limiting illness, end-of-life care, and available services. Caring Info’s goal is for all people to make informed decisions about five types of care: curative, palliative, hospice, comfort, and bereavement.

This is a free resource database of pamphlets, videos, and community guidebooks to assist patients, families, and caregivers with the completion of advance directives. Pamphlets and guidebooks are printable and available in English, Spanish, and Mandarin Chinese.
