Our Services

LATCH Services are provided free-of-charge by telephone, in-home, or in other community locations. Interventions are tailored to meet each patient’s needs while respecting the role of the patient as a decision maker.

Our multidisciplinary Care Management Team is comprised of clinical and non-clinical providers that include a referral specialist, community health workers, health educators, nurse case manager, social workers, disability services coordinator, and SOAR (SSI/SSDI Outreach, Access, and Recovery) case manager. Most of the LATCH team is fluent in English and Spanish.

Our care management helps patients navigate the Durham County health system.

Post-Hospital Follow-Up

Our care managers receive electronic alerts from Duke University Hospital and Duke Regional Hospital. These alerts notify care managers whenever a patient enrolled in LATCH is admitted, discharged, or visits the Emergency Department (ED).

Patient Education

We provide health education to aware and inform patients on a wide range of health topics.

Chronic Disease Management

Targeted chronic diseases include:

  • Asthma and/or Chronic obstructive pulmonary disease (COPD)
  • Congestive Heart Failure
  • Diabetes
  • Hyperlipidemia
  • Hypertension

Psycho-Social Support

We provide referrals to mental health agencies, support groups and pain management.

Access to Benefits

Assistance with Medicaid applications.

Assistance to Medicare Recipients. Our Northern Piedmont Benefits Enrollment Center assists individuals who are in need of additional benefits: Medicare Part D, Extra Help/Low Income Subsidy program; ow-Income Energy Assistance Program; SNAP (food stamps); Medicaid, Medicare Savings program and many other benefits. Bills Assistance

Our care managers inform patients about payment plan options and provide assistance with Charity Care applications.

Interpretation & Translation

The majority of the LATCH team is bilingual in English and Spanish. If time permits, we provide medical interpreting whenever interpreter services are not available.


LATCH provides assistance to patients who do not have their own transportation to get to their medical appointments.

Basic Needs

Referrals to charities and agencies that provide assistance with clothing, food, housing, and utilities.
