The VITAMINS randomized clinical trial: Current surviving sepsis guidelines have a weak recommendation to initiate hydrocortisone in patients with refractory shock. This study tested the hypothesis generated by a single center in Virginia that reported a benefit of adding Vitamin C and thiamine, and gained quite a bit of attention particularly in the lay press despite being low quality evidence. Now, this randomized clinical trial of 216 patients did not show a benefit in terms of days on pressers (the study was not powered to detect a mortality difference).
