About 10% of the time, patients report feeling judged or offended by something they read about in an outpatient note. Content and tone seem to matter a lot. Being mindful of how we create our notes might reduce the frequency of transmission of bias to other clinicians, and it might also reinforce healing, therapeutic relationships.
