This is a really thorough summary. I like how the importance of a physical exam is emphasized, especially the overall appearance of the patient. Consideration is also given as to who should be involved in the visit, ie not just the patient but perhaps family members or other caregivers.
Also, its emphasized that patients be allowed to end the visits, not the providers.
